JOB DESCRIPTION
A Clinic Facilitator is needed to assist with administrative and clinical intake, provide technical support, assist with examination and provide Kaiser benefit information to members and non-members. The retail facilities are also intended to promote KP to non-members, so the facilitator will need to possess strong inter-personal communication skills and be able to interact positively with the general public to explain the benefits of Kaiser, helping guide them to additional information and sales resources.

ROLE DESCRIPTION
Under general supervision and within established scope of practice, the Clinic Facilitator will assist in the provision of direct patient care by performing routine patient care procedures, technical and supportive functions in an employer/retail setting. The Clinical Facilitator will also provide health plan information to KP members, and promote KP to non-members.
QUALIFICATIONS
• CA Nursing License- LVN • 1-2 years of paid work experience in front-line clinical context • Bilingual - English and Spanish • Basic computer skills • Strong inter-personal communication skills • Customer service and/or sales experience preferable • Friendly and engaging demeanor
Responsibilities include: • Utilizes the nursing process to assess, plan, implement and evaluate patient care. • Performs focused interview to identify specific patients needs. • Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition. • Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.
Qualifications include: • Current Licensed Vocational Nurse (LVN) License. • Full command, verbal and written, of the English language. • Current Health Certificate (per facility). • Current PPD or Chest X-Ray. • One year prior professional nursing experience Preferred. • Current BLS card. • Successful completion of the screening tests
